Brief: Inland Revenue called to Account over Data Errors

"Taxpayers held to ransom for Inland Revenue errors" by Tony Collins of Computer Weekly, reveals that the Inland Revenue has discovered huge duplication in national insurance data and placed the onus on individuals to sort it out.

Maybe more of a failure of the system at large - and to do with legacies - than a usability problem per se, but hairy nonetheless.
